Paitent Forms/Update Insurance Information

We recognize that families may experience financial hardship.  To support these situations, Pathology Reference Lab/Pathology Associates of San Antonio offer a Financial Assistance Program to those patients who may be uninsured, under-insured with limited coverage or whose insurance may not have covered testing performed.

Note:  If you were a patient of a hospital and received financial assistance or were approved for a discount for those services, please attach documentation with the approval and covered dates.

Request for Medical Records:

If you would like to request a copy of your medical records, please CLICK HERE.   Complete the form and submit online.   Your records will be sent to you based on the method you requested on the form within 2 weeks.   Note:  You must submit proof of identity for your privacy protection.

Update Insurance:

If you received a billing statement showing incorrect insurance, or your insurance denied for cannot identify your membership, please CLICK HERE to update your insurance.  We will update your account and resubmit the correct insurance.   You can disregard your billing statement until the correct insurance processes your claim.
